Renewal Form Instructions

1. You can renew three ways:

type directly into the pdf (download above) and email the completed form to lbarlow@lesea.org
complete a hardcopy of the form, and mail it to the WHMN offices
fill in the form below and submit.

2. Please answer each question in full.

3. You will receive your new card if your renewal is approved and when payment is made.
